How does evidence become part of how a team works?

A team can have access to data and still make decisions through habit, hierarchy, or selective attention. A culture of evidence is built when leaders make inquiry normal, distinguish confidence from certainty, and create routines in which claims can be examined without turning every disagreement into a contest of status.
